(1) Violators who disagree with the facts of the citation shall file a claim to the charging authority before the final reporting date. If the person filling in this form is entrusted to make the appeal, please provide a letter of proxy. If the responsible authority is not the Office, please do not fill in this statement.

(2) If the driver of the reported case is not the car owner and the car owner desires to clarify the responsibility, the car owner shall, before the final reporting date, provide all required documents (traffic violation notice, evidence photos, driver’s personal stamp, photocopy of driver’s license and car owner’s personal stamp) and apply for it at the Office’s violation penalty window.
